Our non-medical homecare company is experiencing rapid growth and we are seeking an experienced Home & Community Based Services Waiver Specialist to assist with adding value to our current and potential clients. The HCBS Waiver Specialist will assist individuals that inquire about our services supported by the various Medicaid/VA/Waiver programs such as the Adult Family Living Program, Personal Care Services, and Live-In Companion Services, etc. with the application process for getting on the governmentally funded programs. Many individuals do not have the resources or technical ability, or perhaps have other barriers that make this process a challenge. Assisted Living Services, Inc. offers this support service as a benefit to the communities it serves. Our services for assisting with the application, eligibility, and redetermination process governmental programs are complimentary, and clients or prospective clients are made aware that they have the freedom to choose any agency they wish.

The HCBS Waiver Specialist will assist our current and potential clients with the Medicaid/VA/Waiver eligibility process so that they do not lose funding for this much needed benefit. The specialist will work in the finance department and assist clients who are paying privately for home care services to prepare for the eligibility requirements and transfer to the governmental programs when eligible.

The HCBS Waiver Specialist will spend the majority of his/her time in the field visiting clients and potential clients with the application, eligibility, and redetermination process for the Medicaid/VA/Waiver programs. On a regular basis this specialist will work out of our office(s) to complete the tasks assigned for this role.

Reports to: Controller

Salary Range:$50,000-70,000 Annually

Hours of Work: Monday through Friday. Full-time Salaried position. Overtime exempt. Minimum of 40 hours / week.

Essential Responsibilities and Duties:

  • Perform visits and/or telephone consultations with inquirers about services supported by governmental programs.
  • Follow inquirers on a regular basis, offering advice, guidance, and support regarding their eligibility and application status for Medicaid/VA/Waiver programs.
  • Meet regularly with service directors and managers to review client eligibility for governmental programs.
  • Assist clients on “spend down” programs to transition from private pay to Medicaid/VA/Waiver programs to avoid interruptions in service.
  • Create entries in Salesforce and use accordingly to track inquiries.
  • Provide and explain educational materials to interested inquirers and/or professional referral sources.
  • Provide assistance to meet the needs of individual(s)/, family(families), community/communities) through programs funded privately or governmentally.
  • Collaborate with individuals, families, support systems, and other interdisciplinarians to specifically identify problems, needs and how they can be resolved adequately.
  • Responsible for identifying and reviewing documents to ensure that the particular individual remains eligible for programs such as Medicaid/VA/Waiver, etc.
  • Completes other special projects or assignments as requested by Director.
  • Meet and achieve the weekly/monthly benchmarks for assigned Key Performance Indicators (KPIs)

Education and/or Experience:

  • Must possess solid communication, organizational, technology, analytical and management skills.
  • Must like to teach and coach others.
  • Social Work and/or Care Management experience.
  • Must have a bachelor’s or Master’s Degree in social work (or equivalent).
  • Must have valid driver’s license – requires significant in state driving.
  • Proficient computer skills and ability to adapt to technologies that improve services.
